2. Passport type facial photo taken within last six months
3. Proof of immigration status (e.g. Permanent Residence Card or Visa with I-20, DS-2019 or other associated documents)
4. Roundtrip flight itinerary --- Itinerary must include the applicant’s name and date along with the flight information.
(e.g. Florida → Country A → Japan → Country B → Florida)
5. Document to prove the ability to pay for travel expenses (e.g. most recent bank statement, checking or savings)
If someone else pays for travel expenses,
5-1. Document to prove the relationship with the financial guarantor/account holder (e.g. marriage certificate, birth certificate)
5-2. Passport of financial guarantor
6. Detailed day to day travel itinerary describing daily activity plans --- Itinerary must include the name of accommodation, phone number and address
7. Document to prove current residential address (e.g. driver license, state ID, utility bill or lease agreement)
8. Parental consent form for minor applicant and parent's driver liense
