By mail:

This option is only available when the applicant possesses a valid Florida driver’s license or a Florida state issued ID card, which is updated to their current address and for those who reside outside of the Miami-Dade and Broward County.

Please include a copy of your driver’s license or ID card as proof of your residential address in Florida. If the applicant possesses neither of them, he/she is required to apply in person, with a proof of their Florida residential address (e.g. Government mails, apartment lease, utility bill).

Please note that The Consulate General of Japan in Miami is not liable for any mailing incident such as loss, damage, delay, missing, etc. If you would like to avoid any mailing incident, please apply and pick up in person. Also, we strongly recommend you to keep tracking numbers for your records.

If the applicant is not able to apply in person and would like to have someone apply on their behalf, please fill out the authorization form. The authorized person is required to bring the original signed form, and his/her passport or driver’s license.
